Understanding saphenous vein graft patency.

چکیده

Saphenous vein was the conduit used in the first series of coronary surgery, and, with the exception of revascularization of the left anterior descending coronary artery, it remains the most commonly used conduit.1 There are several reasons for this. First, because of its relatively large diameter and wall characteristics, it is technically easy to use; second, it is plentiful, and therefore can be used to perform multiple grafts; third, it is long and can reach any coronary artery; and fourth, it is easily harvested. However, its durability and longevity are not ideal. One year after coronary surgery, 10% to 20% of saphenous vein grafts fail.2–4 From 1 to 5 years, an additional 5% to 10% fail, and from 6 to 10 years, an additional 20% to 25% fail.5 At 10 years, only about half of saphenous vein grafts are patent, and of those, only half are free of angiographic arteriosclerosis.6

Comparison of therapeutic results of large saphenous vein resection with conventional methods and standard bridging

Background: The use of the traditional method for saphenous vein harvesting is associated with wound complications and not on-time patient mobilization. This has caused the improvement of minimally invasive vein harvesting techniques, together with general bridging. This study was designed to compare the therapeutic results of...

Radial artery versus saphenous vein patency randomized trial: five-year angiographic follow-up.

BACKGROUND Graft patency is a fundamental predictor of long-term survival after coronary artery bypass surgery. Left and right internal thoracic artery (arterial) graft patency has been shown to be superior to that of saphenous vein grafts. More recently, the radial artery has been used as an aortocoronary graft, but little is known about the midterm and long-term patency of this conduit. We pe...

Association between Plasma Fibrinogen Level and Saphenous Vein Graft Patency.

BACKGROUND Fibrinogen is related to the pathogenesis of atherosclerosis. The inflammatory process in atherosclerosis may cause an increase in plasma fibrinogen level. Therefore, in this study we proposed to investigate whether plasma fibrinogen is associated with the patency of saphenous vein graft in patients at least 1 year after coronary artery bypass graft (CABG) surgery. METHODS Patients...

Midterm patency rate after saphenous vein grafting with a PAS-Port device.

Abtract: Recently, a new proximal anastomosis device of a saphenous vein graft (SVG) to the aorta, the PAS-Port device (Cardica, Redwood City, Calif) has been introduced and yielded encouraging results in terms of neurologic complications and early patency.1,2 However, there is a concern about the midterm (at least 1 year after surgical intervention) patency rate. The aim of this study was to e...

The no-touch vein graft harvesting technique for coronary artery bypass grafting surgery reduces mortality: a long-term follow-up study

Background The saphenous vein is still the most widely used conduit in CABG surgery despite its poor long term patency. In the 1990’s a new No-touch technique for saphenous vein graft harvesting for CABG was introduced where the vein graft is harvested with a pedicle of surrounding tissue. With the No-touch technique the vein graft does not go into spasm and the vessel wall does not have to be ...

Long-term graft patency (3 years) after coronary artery surgery. Effects of aspirin: results of a VA Cooperative study.

BACKGROUND The long-term success of coronary bypass surgery is dependent on graft patency after surgery. This trial was designed to determine if aspirin improved saphenous vein graft or internal mammary artery (IMA) graft patency between 1 and 3 years after coronary artery bypass grafting (CABG). METHODS AND RESULTS After receiving aspirin 325 mg/d for 1 year after CABG and undergoing a 1-yea...
